Cocamidopropyl betaine (CAPB) is an amphoteric surfactant used in cleansing products. Its contact-allergy literature comes largely from selected patch-test patients and does not establish population prevalence or overall cleanser mildness.

What CAPB is

CAPB is an amphoteric surfactant used in rinse-off personal-care products. Its presence on an ingredient list tells you that it is part of the cleansing system; it does not, by itself, rank the formula's gentleness or predict whether an individual will tolerate it.[1]

What the allergy studies actually tested

A 1997 study began with only 10 people already selected because they had positive CAPB patch tests. Seven reacted during product-use testing; in follow-up testing, six of nine reacted to amidoamine and none reacted to an amidoamine-free CAPB preparation. This supports an impurity hypothesis in that small, preselected group—not a general prevalence estimate.[1]

A larger North American study tested 975 consecutive patients referred for suspected allergic contact dermatitis and compared CAPB with amidoamine reactions. Because this was a specialty patch-test population, its results cannot be translated into the percentage of all cleanser users who are allergic.[2]

CAPB versus manufacturing-related impurities

The literature does not support saying that every CAPB reaction is caused by CAPB itself—or that amidoamine always explains it. Separate patch tests and clinical relevance assessment can matter, while a retail ingredient list usually does not disclose residual impurity levels.[1][3]

What these studies do not tell us

These are allergy and patch-test studies. They do not compare CAPB cleansers with soaps or sulfate systems for cleansing, dryness, barrier disruption, or overall tolerability. Calling a product gentle therefore requires evidence about the complete formula and its use—not merely the presence of CAPB or a sulfate-free label.[2][1]

When recurrent dermatitis needs investigation

Repeated dermatitis associated with cleansers deserves more than ingredient-list guesswork. The relevance study followed patch-positive patients specifically to determine whether test reactions matched real exposures, underscoring why clinician-directed patch testing and exposure history are more informative than self-diagnosing CAPB allergy.[4][3]

Is CAPB allergy common in the general population?
The cited studies cannot answer that. Their participants were referred patch-test patients or people already known to have a positive CAPB test, so their findings should not be converted into general-population prevalence.[1][2]
Is every CAPB reaction caused by the CAPB molecule itself?
No. Some studies implicate amidoamine or other manufacturing-related material in at least some reactions, but the exact relevant allergen can vary. Separate testing and clinical interpretation may be needed.[1][2][3]
Does CAPB or a sulfate-free label prove a cleanser is gentle?
No. The cited CAPB studies evaluate contact allergy, not overall cleanser mildness or comparisons with alternative surfactant systems. They cannot prove that a finished cleanser is gentle from CAPB or sulfate-free labeling alone.[1][2]
